Following the death of "Glee" star Cory Monteith new details have been released about how the star passed prior to his overdose in a Vancouver hotel. The BC Coroners Service in Victoria, British Columbia have released the final report on the July 13th tragedy.

The actor who played Finn Hudson on the hit Fox series was found dead in his Fairmont Pacific Rim hotel room in Canada at age 31. Monteith's body was discovered when the actor missed his planned check out time. Hotel staff then paid a visit to the room finding him unresponsive, according to Us Weekly.

Inside the room drug paraphernalia was found "including a spoon with drug residue and a used hypodermic needle," the report read. Also present were two empty bottles of champagne. Monteith was last seen via hotel video at 2:16 a.m. on July 13th returning to his room alone. "He was not seen alive again," according to the report.

At the time of his death the actor was dating his "Glee" costar Lea Michele. The actress along with fans and other loved ones were all aware that the actor previously abused drugs.

The coroner's report confirmed that Monteith "had a history of illicit drug use with intermittent periods of rehabilitation and of abstinence from drugs."

The actor abused heroin among other substances but had completed a rehab program this past spring. The final report stated, "It is noted that if someone has abstained from using opioids (which include heroin) for a period of time, tolerance may be lessened."

On Thursday, October 10th "Glee" will honor Cory Monteith and his character, Finn Hudson with a special tribute episode titled "The Quarterback."
